Our services are limited to the United States. We accept Medicare, Workers
Compensation, and most Major
Insurance
carriers. Patients are responsible
for all referrals, deductibles, out of network fees, signed prescriptions, signed
certificate of medical necessity forms, signed delivery sheets, down payments,
house call payments and non-covered items. If
we tell you that an item might not be covered by your insurance, we will require
full payment for that item before fabrication can begin. You will then be
responsible for collecting the reimbursement from your insurance carrier on that
item. We will be glad to assist you with any paperwork needed. We do not accept Medicaid
unless pre-approved.
