Medicare: Questions Remain on O&P Coverage

Both Democrats and Republicans both agree that there is no defined coverage in the new Health Care and Modernization Act. One certainty is that there will be cuts in the current coverage for both Medicare and state sponsored Medicaid.

 

"Watching the news can scare the socks off anyone that depends on O&P products and services" says: M. Britt Spears, President of Spears Prosthetics and Orthotics/Rehab Services in Memphis, TN. "Trying to balance the budget appears to place the O&P coverage as well as regular medical care on the chopping block. There is already a 5 year rule involving Medicare and Orthotic products. That sets limits on services unless there is a documented medical change in condition. A medical change in condition can be defined as: significant weight/volume gain or loss, increase or decrease in functional activity level or any other change that effects independence. The next logical alternative is to place the 5 year rule on Prosthetics and reduce coverage. We are stepping up our commitment to patients by providing complimentary evaluations for their O&P needs. These evaluations will allow us to deliver services before any reduction in services can take place."

With Osteoarthritis, joints tend fall out of alignment. This mis-alignment whenever present in the knee causes the joint to erode the bone surfaces. The erosion will cause pain during weight bearing. With the population starting to age, osteoarthritis is becoming more prevalent.

 

How does this brace work?? The Unloader shifts the knee back into the correct alignment applying pressures both above and below the joints. This pressure reduces the angular vertical load thru the knee and places the knee into the correct alignment. By placing the knee in the proper alignment, there is a huge decrease in pain during weight bearing.

Reminder that our complimentary Amputee Clinic is still open on Monday Feb. 28, 2011. We will examine the fit and function of your prosthesis. We will also discuss with you the newest technology available including Bionic Technology. We can assist with any Rx or additional paperwork including insurance coverage and out of pocket costs. This clinic is open to all amputees no matter who has provided the prosthetic services.

We have our in-house fabricated LSO and TLSO braces readily available and never needed to be ordered. This helps speed up discharge times for our local hospitals. These have Medicare, Medicaid and private insurance coverage.

 

We have contracts and can provide in-patient services for the following hospitals:

CMS has established Internet-based Provider Enrollment, Chain and Ownership System (PECOS) 

Physicians, non-physician practitioners and provider and supplier organizations must enroll and be listed in order to prescribe Orthotic- Prosthetis items. Medicare enrollment can be performed via the internet.

We have a trial Proprio Bionic Foot in our Memphis office. This will allow us to try Bionic technology for amputee patients. Bionic Technology is the marriage of computers to Prosthetic hardware to create an actual Artificial Intelligence that can automatically-in milliseconds, sense and react to the amputee's encounters during everyday walking. This creates a much more stable interaction between the user and walking situations. It creates it's own walking strategies for many different events. It has been proven to eliminate both falls and skin breakdown in unilateral and bilateral amputees.  Instills much more confidence in the users abilities and allows the user to start noticing the beauty of nature instead of mentally concentrating on pathways to navigate.If qualified, Medicare, Veterans Administration, Cigna and many private insurance companies do cover the Proprio Bionic Foot and other Bionic items.  

 

Watch the video below notice how the toe of the foot actually lifts up while walking. This demonstration shows how our patient is able to walk straight up and down inclines. This is because the Proprio foot/anke anticipates the next action during walking. The foot portion is actually a Flex-Foot and has energy storage capabilities. It has a cosmetic foot shell that fits firmly inside a shoe.

               Worlds 1st Prosthesis??

  
This is a recently discovered prosthetic toe. It was found still attached inside the still wrapped Egyptian mummy.

 

Notice the detail and craftmanship.    
  

 The wood and leather prosthesis dates from 1069 to 664 B.C., based on artifacts it was found with in the mummy's burial chamber. This means it predates what was previously thought of as the earliest known functioning prosthesis, the Roman Capua Leg, a bronze artifact dating from about 300 B.C. The leg was once at the Royal College of Surgeons in London but was destroyed by bombing during World War II. 

 

Scientists have found a variety of artificial body parts restored on mummies, including feet, legs, noses, ears. 

                           Accreditation Update:

  
We have just completed our ABC Accreditation site visit. This is part of the Accreditation process required by Medicare. At this time we are the ONLY ABC Accredited Orthotic-Prosthetic-Mastectomy care facility in both Shelby county Tennessee and Cleburne county Arkansas.
  
Last year we had the highest score in the Mid-South area for O&P facilities.

Medicare Changes for 2011 Deductable: $162.00 

With the start of 2011, Medicare and many private insurers have made several changes that will effect coverage for many items. Many items have either been dropped from coverage or their coverage has become very difficult on the medical necessity requirements. 

One major change is in the medical necessity requirements for diabetic shoes and inserts. The process is required to occur in this order:
  1. Have their Physician treating their diabetes physically examine their feet. Noting specific areas of concerns such as, callous formations, peripheral neuropathy, history of amputation, etc.
  2. The Physician then must write a treatment plan that includes the order for diabetic shoes and inserts. This must fulfill the medical necessity requirements. They then can refer the patient to our facility. We must receive the written examination and orders.
  3. We then have to examine the feet and send back a detailed Rx to the same Physician.
  4. Once we receive the signed Rx, we are then able to follow through with measurements and order the shoes.

This Month's Recipe: From The Medieval Cookbook

Samon roste in Sauce
Take a Salmond, and cut him rounde, chyne and all, and roste the pieces on a gredire; And take wyne, and pouder of Canell, and drawe it thorgh a streynour; And take smale myced oynons, and caste there-to, and let hem boyle; And then take vynegre or vergeous, and pouder ginger, and cast there-to; and then ley the samon in a dissh, and cast the strip theron al hote, and serue it forth.
275 ml/10 fl oz/1 1/4 cups medium-dry fruity white wine 175 g/6 oz small onions, peeled and finely chopped Good pinch of ground cinnamon 1/4 teaspoon white wine vinegar Good pinch of ground ginger 6 salmon cutlets, about 2.
5 cm/1 inch thick Oil for grilling  No oil or fat is mentioned in the original recipe, but they are suggested for frying other fish in the same manuscript.
Fish must be grilled on a greased surface whether cooked over the heat or under it as we cook it now.
Oil was the obvious medium to use, because salmon could be eaten at Lenten meals, when strictly pious people did not eat butter.
We do not need to 'streyne' our wine or spices today.
Cook the wine, onions and cinnamon gently in an open pan until the onions are soft and the wine is slightly reduced.
Add the vinegar and ginger, and leave at the side of the stove.
Heat the grill while you brush the fish cutlets with oil on both sides.
Grill them under moderate heat, turning once, until just cooked.
Serve each cutlet with a spoonful of wine and onions on top.
